Pressure injury prevention Text A The Braden Scale scores six areas: sensory perception, moisture, activity, mobility, nutrition, and friction and shear. Totals range from 6 to 23. A score of 12 or below means high risk, 13 to 14 moderate risk, and 15 to 18 mild risk. Reassess on admission, then every 48 hours or after any change in condition. Text B High risk, in bed: reposition at least every 2 hours using a 30-degree tilt. High risk, seated: limit sitting to 1 hour at a time. Moderate risk: reposition at least every 4 hours. Record each change of position on the turning chart. Keep heels clear of the mattress with a pillow under the calves. Text C Foam dressings suit wounds with light to moderate exudate and can stay in place for up to 7 days. For heavy exudate, use an alginate dressing and change it when saturated. Film dressings protect intact skin over bony areas but must not be used on infected wounds. Text D Check your skin every day, especially your heels, hips and the base of your spine. Tell a nurse about any red patch that does not fade within 30 minutes. If you use a wheelchair, lift or shift your weight every 15 minutes. In which text can you find the answer to each question?
