A pain review and fear of being labelled supplied plan
- A patient has avoided using the call bell to discuss pain because a previous request felt dismissed. The nurse can gather the current account and request review under the ward's documented plan.
- The nurse can ask about the patient's current experience, its effect and what has already been discussed, then document the account.
- The nurse will follow the ward's existing assessment and review process; any medication decision remains with the authorised clinical team.
- Do not recommend a dose, promise complete relief or dismiss a new report because an earlier review occurred.
- First, please describe what you feel now, when it changes and what it stops you doing. I can record that alongside what has already been discussed, rather than rely only on the earlier review.
- Your ward plan says a new pain report should be assessed and the current plan reviewed with the responsible team. I will follow that process; I cannot choose a dose or promise complete relief here.
