Medicine-list clarification process

The current discharge list and the older display copy must be compared with the medicines available at home.
Any discrepancy goes to the prescriber for clarification; no candidate is asked to choose a dose or medicine.
Until clarification, the nurse must not advise an independent prescription change.
Once confirmed, provide the approved current list in large print and separate the superseded display copy with consent.
Patient wants to retain responsibility; daughter may help identify the current list, with permission.
