A 52-year-old woman is being treated for a kidney infection. Which finding most strongly indicates that the infection has become life threatening?
How hard is the EMT exam?
Most candidates pass, but the exam feels hard by design. An adaptive test keeps asking questions near the edge of what you know, so nearly everyone leaves unsure how it went.
| Exams passed, 2025 | 80% of 132,934 exams delivered |
|---|---|
| First attempt, by state | 57% to 88% in 2025 |
| Partial credit | None: every item is right or wrong |
| Pace | About 1 minute an item if the exam runs to 120 |
Why it feels hard
- The more items you get right, the harder the next ones become, so you will meet questions you are unsure of whatever your level.
- Most items are scenarios that ask what to do first or next. Several options are reasonable; only one fits the priority order.
- Multiple-response, ordering and sorting items give no credit for being partly right.
- Primary assessment is 39% to 43% of the exam, so gaps in airway, breathing, circulation and recognizing life threats show up again and again.
What the numbers say
In 2025 the National Registry delivered 132,934 EMT certification exams, and 80% of them were passes, counting every attempt. First-attempt pass rates by state ran from 57% to 88%.
How to check you are ready
Take a timed practice test and look at your result by domain, not just the total. Fix the weakest domain first, then sit a full 120-item practice exam on the 2-hour clock so the longest possible sitting holds no surprises.

An 88-year-old woman tells you she is 'just a bit tired' and does not want a fuss. As you approach, you notice she is gray, sweating and breathing fast. How should your general impression guide you?
You are holding manual stabilization of a driver's head from the passenger seat. The steering-wheel airbag did not deploy. How should you position yourself?
